Substandard instruments being used in operations by the NHS who is responsible?

A top surgeon who wants to remain anonymous has revealed how substandard instruments are being used routinely in operations. Should the surgeons refuse to use these instruments on patients?. I feel they are just as much to blame as the Government by carrying on performing operations with bad equipment.
